问题： 临床上碰到有些病人，有明显的psychiatric problems, 不能verbalize his situations and understanding, 有的有mental retardation, doesn't seem to understand their treatment options. 但他们是合法的 their own decision maker per psychiatrist, 有的还有court order. 一直很奇怪，你们是怎么评估这类病人的？

答：过去是按DSM-IV来评估。去年10月1日开始全美是按DSM-5来评估。
